Tesamorelin is the full 44-amino-acid sequence of human GHRH with a small chemical modification at the N-terminus that protects it from enzymatic breakdown. Unlike most research peptides, it has a substantial clinical record: it is approved in some jurisdictions, including the United States, for a specific clinical indication, which gives it the most robust human dataset among GHRH analogues.
Falutz et al. (2007, N Engl J Med) reported a randomized, placebo-controlled trial in which tesamorelin reduced visceral abdominal fat in a specific patient population, with increases in IGF-1. Follow-up trials examined liver fat and body composition endpoints.
As a GHRH analogue it amplifies the natural pulsatile release of GH rather than replacing GH, a distinction researchers use to study the axis.
