Survodutide vs Retatrutide (2026): Which Glucagon Drug Wins?

The short version

Survodutide and retatrutide are cousins. Both add a glucagon “fat-burning” arm on top of the GLP-1 appetite pathway — but retatrutide goes one further and adds GIP, making it a triple agonist, while survodutide stays a dual agonist. That extra arm is why retatrutide has posted the bigger weight-loss numbers (low-20s percent vs survodutide’s ~16.6%).

But weight is not the whole contest. Survodutide’s standout result is a reported 63% cut in liver fat, and it is aimed squarely at fatty-liver disease. Retatrutide is chasing the broadest obesity market. Neither is approved. Here is how they really compare.

Two unbranded medication pens side by side

The quick answer

If you only remember one thing: retatrutide is the stronger scale-mover, survodutide is the liver specialist. They share a glucagon arm, so they are more alike than either is to plain semaglutide, but they were built with different finish lines in mind.

Retatrutide (Eli Lilly) is a triple agonist — GIP, GLP-1 and glucagon — and its phase 3 TRIUMPH programme produced some of the largest weight-loss figures ever reported for an obesity drug. Survodutide (Boehringer Ingelheim and Zealand Pharma) is a dual GLP-1/glucagon agonist whose weight-loss number is a step lower but whose effect on liver and visceral fat is dramatic. We cover each on its own in what is retatrutide and what is survodutide.

Same glucagon engine, different gearing. Retatrutide tuned it for maximum weight loss; survodutide tuned it for the liver.Survodutide vs retatrutide, in a line

Dual vs triple: the mechanism difference

Both drugs start from the same two ideas. GLP-1 reduces appetite and slows digestion. Glucagon-receptor activation raises energy expenditure and pushes the body to burn stored fat, including in the liver. The difference is the third receptor.

  • Survodutide = GLP-1 + glucagon (dual). Two levers: eat less, burn more.
  • Retatrutide = GIP + GLP-1 + glucagon (triple). It adds GIP, a second incretin hormone that works with GLP-1 on appetite and insulin — the same target that helps make tirzepatide so effective.

More targets is the reason to expect more weight loss, and in practice that is what the trials show. But more targets can also mean more to balance on tolerability, which is why the glucagon class as a whole is watched closely at its highest doses.

Weight loss, side by side

These numbers come from different trials with different patients, so read them as directional rather than a true head-to-head.

On raw weight loss, retatrutide wins this comparison — its triple-agonist design simply moves the scale more. If a bigger number is the only thing you care about, retatrutide is the headline. But that framing misses what survodutide was built to do.

Beyond the scale

This is survodutide’s whole argument. Around mid-2026, Boehringer reported a pre-specified analysis showing a 63% reduction in liver fat and a 34% reduction in visceral fat (the fat around the organs), while “minimizing lean mass loss.” Survodutide is in dedicated trials for MASH, a fatty-liver disease with very few treatments.

Retatrutide is not ignoring metabolic health either — its glucagon arm also cuts liver fat, and Lilly’s global filing plan covers obesity plus knee osteoarthritis pain and obstructive sleep apnea. But its centre of gravity is the broad obesity market, where its numbers are unmatched. Survodutide’s centre of gravity is the liver.

Ask “which loses more weight?” and retatrutide wins. Ask “which fixes a fatty liver?” and survodutide is the one built for the job.The real fork in the road

Side effects

Both are GLP-1-based, so the common side effects are the familiar gastrointestinal ones: nausea, vomiting and diarrhea, worst during dose escalation and easing over time.

The glucagon arm they share draws the extra scrutiny. In retatrutide’s TRIUMPH-3, diarrhea hit about 30.1% at the 9 mg dose versus 8.7% on placebo, and discontinuation for adverse events ran 13.5% at the top 12 mg dose — the biggest weight loss came with real tolerability costs. Survodutide’s profile is broadly similar in kind (GI-led), and because both touch glucagon, blood-sugar and heart-rate effects are monitored closely. Neither has established long-term human safety yet; both are investigational. For the retatrutide detail, see retatrutide side effects. None of this is medical advice.

Which is further ahead

On the road to approval, retatrutide is the more advanced obesity candidate: its phase 3 programme is complete and Lilly plans to file with the FDA in the first quarter of 2027, which realistically points to approval in late 2027 or later. Survodutide’s obesity and MASH trials are still reading out through and beyond 2026, with filings to follow.

So neither is close to a pharmacy shelf, and both are years from Canada if they arrive at all. Anyone comparing them today is comparing trial data, not products you can buy. For the broader timeline, see the next weight-loss drugs after Ozempic and CagriSema vs retatrutide.

Myths

“Myth: Retatrutide is just a stronger survodutide.”

They overlap on GLP-1 and glucagon, but retatrutide adds GIP and targets the general obesity market, while survodutide is tuned for liver disease. Different third ingredient, different mission — not simply more or less of the same thing.

“Myth: Survodutide failed because it only hit 16.6%.”

16.6% is a strong weight-loss result, comparable to injectable semaglutide. Judged on its real target — liver and visceral fat — its numbers are striking. It did not “fail”; it is aimed at a different scoreboard.

“Myth: One of them is available if you look hard enough.”

Neither is approved or sold as a medicine anywhere. In Canada, unauthorized injectable peptides sold online are illegal, and a “research use only” label changes nothing about that.

“Myth: The triple agonist is automatically safer or better.”

More targets can mean more effect but also more to balance. Retatrutide’s top-dose tolerability costs are a reminder that “more receptors” is not a free upgrade.

For researchers

Neither survodutide nor retatrutide is something to source for human use — both are investigational drugs. For laboratory pharmacology work on incretin and glucagon-receptor biology, TrueLabs supplies reference peptides in CAD, for in vitro and laboratory research only, never for human or veterinary use, including RT3 (retatrutide) from $90, TZ-2 (tirzepatide) from $73 and cagrilintide from $40. Check identity and purity against a lot-matched certificate — see how to read a peptide COA and the COA library.

FAQ

Tap a question to reveal the answer.

Is survodutide or retatrutide better?

For raw weight loss, retatrutide — its triple-agonist design produced low-20s percent figures versus survodutide’s ~16.6%. For fatty-liver disease and visceral fat, survodutide is the one built for the job, with a reported 63% liver-fat reduction. They target different goals.

What is the difference between survodutide and retatrutide?

Survodutide is a dual GLP-1/glucagon agonist from Boehringer Ingelheim. Retatrutide is a triple GIP/GLP-1/glucagon agonist from Eli Lilly. Both share the glucagon arm, but retatrutide adds GIP, which helps explain its larger weight-loss numbers.

Do they have the same side effects?

Broadly yes — both are GLP-1-based with gastrointestinal effects (nausea, vomiting, diarrhea) leading. The shared glucagon arm means blood-sugar and heart-rate effects are watched. Retatrutide showed notable diarrhea and discontinuation at its highest dose in TRIUMPH-3.

Which is closer to approval?

Retatrutide for obesity: phase 3 is complete and Lilly plans to file with the FDA in Q1 2027. Survodutide’s obesity and MASH trials are still reading out. Neither is approved as of October 2026.

Are either available in Canada?

No. Neither is approved or sold in Canada. Health Canada’s 2026 advisory warns that unauthorized injectable peptides bought online are illegal and that research-use labels give no exemption.

Why does survodutide target the liver?

Its glucagon arm drives the liver to break down stored fat. Survodutide reported a 63% reduction in liver fat and is in dedicated trials for MASH, a fatty-liver disease with few treatments — a market retatrutide is not primarily aimed at.

Is retatrutide a “better survodutide”?

No. They share two receptors, but retatrutide adds GIP and chases the broad obesity market, while survodutide is tuned for liver disease. Different design and different mission, not a simple upgrade.

Can I use either for weight loss now?

Neither is an approved medicine. Anyone considering weight-loss treatment should talk to a doctor about options that are actually authorized, rather than sourcing investigational or research material.
